Zenwork Inc. ("we," "us," "our") is dedicated to providing you with a secure and transparent process for accessing your tax information from the Internal Revenue Service (“IRS”) using Form 8821. This consent form outlines the procedures, authorizations, and disclosures involved in obtaining your tax information and sharing it with DeLeon & Stang CPA and Advisors ("DeLeon & Stang CPA and Advisors"). 1. Authorization for Tax Information Retrieval:
By signing below, you grant our authorized representatives the authority to request and receive your IRS account transcripts and tax return information (collectively referred to as "Tax Information") for the time period indicated below ("Tax Period").
2. Tax Information Retrieval Scope:
Your Tax Information may encompass a variety of IRS forms, including but not limited to 1040, 1120, 1065, 990, 940, 941, Wage & Income, Civil Penalty, and 1040 Separate Assessments. This retrieval aims to provide a comprehensive picture of your financial history for the specified Tax Period.

3. Duration and Revocation of Authorization:
This authorization remains valid for three years or until you choose to revoke it. If you decide to revoke this authorization, you may do so in writing. However, please note that any information released before the revocation will remain valid. To initiate a revocation, kindly contact Zenwork at privacy@zenwork.com.
4. Consent for Disclosure to DeLeon & Stang CPA and Advisors:
With your approval, Zenwork is authorized to share the Tax Information obtained with DeLeon & Stang CPA and Advisors, solely for the purpose of [Specify Purpose, e.g., "assessing your eligibility for a financial transaction with DeLeon & Stang CPA and Advisors"]. Your consent empowers us to disclose relevant Tax Information as required for the aforementioned Purpose.
5. Protection and Usage of Tax Information:
Zenwork operates under the premise that your Tax Information is private and sensitive. Except as legally required or authorized by you, Zenwork will not disclose your Tax Information. While federal law necessitates this consent for disclosure, please be aware that once disclosed, your Tax Information may not be protected against further use or distribution.
6. Reporting Unauthorized Usage:
If you suspect that your tax return information has been mishandled, disclosed improperly, or used without your consent, please contact the appropriate authorities. The Treasury Inspector General for Tax Administration (TIGTA) can be reached at 1-800-366-4484 or via email at complaints@tigta.treas.gov.
7. Electronic Signature and Consent:
By submitting this form, you confirm your understanding of the procedures outlined above and grant Zenwork the authority to electronically sign and file IRS Forms 4506 and 8821 to collect your Tax Information and share it with DeLeon & Stang CPA and Advisors for the duration indicated below ("Authorized Disclosure Period"). If you are a corporate officer, partner, guardian, executor, receiver, administrator, trustee, or party other than the taxpayer, you certify that possess the necessary authority to electronically sign and grant consent on behalf of taxpayer.
